In short. Observation, sun protection, energy-based and injected treatments, muscle-relaxing injections, fat removal, then a lift. Only the last removes skin.
Key points
- A course of tightening treatments changes how the skin feels and looks, and it does not change how much of it there is.
- A neck that drapes will still drape after the course has finished, and the honest expectation is a change in quality rather than in outline.
- That distinction is the whole reason this page exists.
There is a ladder of options for a neck, and the matching matters more than the order. Each rung answers one tissue, and only the top one removes skin.
Observation
The first rung is to be assessed, know which tissue is loose, and do nothing. That is a complete answer where the change is mild, where the skin still holds, where the weight is still moving, or where the appearance does not trouble you. It also leaves every other rung available, which is not true of anything already done.
Sun protection and skin care
Protection from sun and ordinary skin care preserve the quality of the skin you have and slow further loss of recoil. They tighten nothing. They are worth doing on their own account, and they are also the preparation that makes any later plan safer and any scar easier to manage.
Energy-based and injected treatments
These improve skin quality and surface texture, and produce a modest change in looseness. Injected treatments that act on skin quality sit in the same group. None of them removes skin, and a neck with a true excess of it has a ceiling these cannot pass. Where texture rather than a fold is the complaint, they are aimed at the right thing.
The rung that is often mistaken for a lift
A course of tightening treatments changes how the skin feels and looks, and it does not change how much of it there is. A neck that drapes will still drape after the course has finished, and the honest expectation is a change in quality rather than in outline. That distinction is the whole reason this page exists.
Muscle-relaxing injections
These soften platysma bands while the effect lasts. They are temporary and repeated rather than a correction, and they answer a band rather than a covering. They are relevant where cords on activation are the finding, and not relevant where the problem is skin.
Fat removal under the chin
Where fat is the main contributor and the skin still recoils, removing it changes the angle between the chin and the neck. Where the skin has already lost its recoil, removing fat from beneath it leaves more skin relative to what is under it, and the neck can look looser afterwards. The two situations are told apart by examination.
A lift
The top rung repositions the muscle and removes skin, and it is the only rung that changes a neck whose covering no longer fits. It leaves a scar and a recovery measured in weeks, and the result is judged months later rather than in the first days. Above it there is nothing: non-surgical options change skin quality, and none of them removes skin.
Do not work through the ladder if there is
- A new or changing lump in the neck, or one that does not move on swallowing.
- Difficulty swallowing, a persistent voice change, or difficulty breathing.
- Swelling on one side only, or a neck that has become tender and red.
What the non-surgical rungs cannot do is set out in What Non-Surgical Neck Treatments Do Not Remove. What keeps a result over the years is explained in Maintaining a Neck Result, and What Reverses It.
Frequently asked questions
Where does the ladder usually start for a neck?
With an assessment and, very often, with nothing. If the finding is a band that appears on activation and the skin still holds, the relevant rungs are few and specific. If the finding is skin that drapes, the honest answer is that the rungs below the top one cannot change the amount of skin, and that is said at the assessment rather than after a course.
How long do muscle-relaxing injections last?
Their effect is temporary, and it is repeated rather than corrected. The bands soften while the effect lasts and return as it wears off, which is why the rung is described as maintenance of an appearance rather than as a correction of the muscle. Your own plan and interval may differ and should be discussed.
Is fat removal ever the right first step?
Where fat under the chin is the main contributor and the skin still holds, it can be. Where the skin has lost its recoil, removing fat from beneath it leaves more skin relative to what is under it and can make the neck look looser. The assessment places the fat against the skin and the muscle before that decision is made.
Risks & Limitations
- Non-surgical treatment improves skin quality and removes no skin.
- Muscle-relaxing injections soften platysma bands while the effect lasts, and then wear off.
- The neck continues to age after any procedure, and further weight change alters a result.
- Scars from a lift are long and do not disappear. They mature over 6–12 months.
When to Seek Care
- A new or changing lump in the neck, or one that does not move on swallowing.
- Difficulty swallowing, a persistent voice change, or difficulty breathing.
- Swelling on one side only, or a neck that has become red, hot or painful.
- Difficulty breathing or chest pain — seek emergency care immediately.
